proportions of displayed page

In Word 2003 the shape of a page as displayed on the screen (both in editing
mode and in print preview) is elongated. A displayed A4 page looks taller and
narrower than in real life. Is this usual? Can it be adjusted?
 
Word could be reserving space for the track changes/comments balloons.
Have you tried turning them off in Tools | Options, Track Changes tab?

No, both balloon space and hiding "white space" result in a document that is
shorter and wider than normal, not taller and thinner.

Is the problem with all documents? Then, reinstalling the printer
driver could be worth trying.

If the problem is with a specific document, and the settings in File |
Page Setup (margins, paper size, etc.) seem correct, then you could
try the following: On the Format menu, click Columns. Click the "One"
option, even if it already appears to be selected, and click OK.

Is this just in Word, or everything (and perhaps you just notice it in
Word)?

If you run a modern 5:4 ratio monitor at a 4:3 resolution (1024*768 perhaps)
you will see this effect.
