How do I build form in Word to be able print (data only) onto a 3 part
colored form on a dot matrix printer.

Use a bordeless table with fixed cell sizes to position form fields to match
the positions of the printed form (a full size scan of the form placed
behind text will help layout the table). Check tools > options > print >
print data only for forms. You can select the background image with the
white arrow on the drawing toolbar to delete it when you have finished with
it.

Thanks you Graham, I'll give that a try.

Hello Graham
I have tried to work thru your suggestion, but was unsuccessful. Can get the
full size scan to position behind the text for alignment. You can tell I'm a
novice. Any suggestions?

Select it to display the Picture toolbar. Click on the Text Wrapping button
(dog icon) and choose Behind Text.
