I want to print four forms on one sheet of paper (about the size of a index
card) or at least two per page. How do I do that. I can only get it to
print one per page?

Forms are not designed for printing and don't really offer all the controls
to print out in special ways.

Build a report and print that. Sounds like you are basically making
"labels" I think there is a standard "post card" label that will print four
per page.
