PPT forms

Hi,

I'm doing an inter-active presentation which is distributed via CD with ppt
viewer and also impatised for the web. On this presentaion I have a contact
page. Is there any way to put fields i.e. name, tel number, text box where
the viewer can
fill these in just like on the web and submit it via an e-mail. If so will
this work with the viewer and on the web. Any suggestions or tips will be
really appreciated.

With PowerPoint, this requires VBA scripting, and VBA does not work in
the Viewer or over the Web.
