Poster with tear-off phone numbers

I am sure that many of you have seen the advertising posters that have
at the bottom phone numbers to be torn off.
The text is usually horizontal in portrait orientation while the phone
number slips at the bottom of the page are vertical.
I would also like to have on the slips my name, say in light gray and
on top of it the phone number in bold black.
Is there maybe a template for something like that?
If not can you give some tips?

Publisher includes many templates for this specific purpose. To replicate it
in Word, use a table, with borders or not as desired. You can rotate text in
table cells.
