Pop-Up Form According To Date

Greetings,

I need to be able to have form pop-up after a certain date have been entered into a field. For example, Monday (in short date form) has been entered. Mondays form needs to popup so that the user can pick a time slot for that day. There would be a form per day listing times slots for that perticular day.

Also The time slots (in the form) need only to be used ONCE in that day, eg, once a time has been entered for that day it cannot be entered or used again, for that day.

Here's hoping to get some answer, even if it's IDK.
