Please help me for this matter (One Date and More records)

Please Anyone tell how to prepare a form with one date and One Route
just like this

In this form I want to enter the date only one time then I could be
able to enter other details under that date and Route. Please help me
to creat a form like above and how do I want to creat table and Queries

Hi,
Create 2 tables for route and route details with required fields and then
run Form wizard - it will help you to make such form.
