Personal Folders Backup add-in installed but no Backup item in File menu

Make sure that the add-on is enabled in Tools-> Options-> Other-> Advanced
Options...
Otherwise try renaming outcmd.dat to outcmd.old. This will reset your
Toolbar.

I thank you both for your suggestions. Unfortunately, I have been
unable to find the add-in (since I don't know what name it was given),
and the renaming of the outcmd.dat file did not do the trick. I think
I'll go back to using the backup utility. I just get nervous that if
I backup while Outlook is running, the backup file may be corrupt.
Sure, I know, shut down Outlook! But I was hoping for a more
automated, fool-proof method.
