Provided you have the version of Adobe's add-in that works properly with
Word 2003, you can set the option from the Adobe menu (change conversion
settings) to save bookmarks. This only works when you use the add-in to
create the PDF. Your original Word bookmark names will probably not be
retained, but if you bookmark the whole heading the heading content will be
used as the bookmark.
