Parameter form not working?

For some reason, my parameter form doesn't seem to be working. I have set it
up correctly (I have another DB with a parameter form working correctly), but
when I try to pull up the query with my data in the combo box, all I get in
the query is 1 record completely blank. I do have data that should display
in the query. I have even tried to just open the query and enter in my data
via the parameter box that opens and it works. Just, for some reason, the
form is not passing the information in the combo box to the query.

Does anyone have any idea why this wouldn't work? I have re-created the
form and the query multiple times and nothing will work.
