We are running a Windows 2003 domain with a 2003 Exchange server. I have 2
users who have to put in their user ID, password and domain every time they
open Outlook. I have deleted and recreated their Outlook, with no success.
Any ideas?

Not sure if this would apply to you, but I have seen this with Office 2003
when users are set up with RPC over HTTP(S) - and it's set to always try
connecting over HTTP first, then TCP/IP. Take a look under the email account,
more settings, Connection to see if Connect to my Exchange mailbox using HTTP
is checked. Not as common yet in today's world, so not sure if this would be
the case for you.

We are running Office 2002, I did check the connection settings and they are
set correctly.

Don't know why but I was running into the same problem. First make sure save
password is checked. Then, make sure the user has the correct permissions to
access the folder where the pst file is located. But what fixed this issue
for me was to create another location for the data file to be stored, then
make the new location default, and delete the old location (BUT, make sure
you have a backup of your pst so it doesn't delete it when you remove the old
data file.

I don't know why this worked for me but it did, so you may want to give it a
try. Hope it helps!
