Outlook randomly inserts attachments in body or attachment line in header

Outlook randomly inserts attachments in body or
attachment line in header.

Why is that and is there a way to override this so that
attachments always appear as a line in the header portion
rather than appear in the body of the message.

Using Outlook 2002 SP-1 on Win2k SP-3 build 2195

Where the attachments sits is determined by the message format, Rich text in
body of message, Plain and HTML in its own field
