Outlook password checking

For some reason my Email account information changes. I
find out it has been changed when I send/receive. A
dialog box with a username/password pops up and the
server is changed to localhost and the username is
changed to <username>/<servername>. Also, once my mail
has been sent and received successfully it still asks me
for this password.

It is not Outlook that is doing this but your AV or firewall that you have
set to check incoming mail. Uncheck this option and the annoying behavior
will go away.
