Outlook not including attachment when forwarding an email

Hello everyone, one of my users is having a weird issue that i'm hoping
someone might have some ideas on, i would greatly appreciate it. As the
heading of my question implies, when this user tries to forward an email with
an attachment, outlook is opening the forward email window without the
attachment in it. Almost as if there's is a direct setting telling outlook
to delete the attachment out of an email before forwarding it. There is no
setting i know of that tells outlook to disclude attachments when forwarding.
I'm at my wits end, 6 years in IT i've never seen this issue. I've seen
where the recipient doesn't get it because outlook blocks it (like with a
..exe or .pst file). Anyway i'm hoping someone in here might know what is
causing this. Thank you.

I aplogize, i forgot to add that this is outlook 2003. Also, it's all
emails with attachments, not a paticular one. The user also reports that she
gets no error message.

The user is saying that outlook is doing this with all emails with
attachments, hence attachments of all types (word, excel, pdf, etc.). As far
as AV we use Symantec AV in a server/client scenario. It's not likely SAV is
interferring though. I was hoping there was in fact a setting in outlook i
don't know about that could be causing this. If this is not the case I will
have to start looking outside Outlook for the cause. If you have any other
suggestions i would greatly appreciate them, and thank you for your reply!
