Outlook Outlook Forms Problem

Hello,

I am new to outlook forms. I have created a form that I wish to fill in and send to recipients within the company. We do not use an exchange server so I am wondering how the recipients see my form.

When I send the form the recipient sees a blank email.

How do I resolve this?
