Outlook form or template?

I've been asked to put together a message/questionnaire that can be e-mailed
to clients. The questions would be answered with a graded system ( 1 to 5).
I could do this with a form but I'm wondering what the best format is so
that most e-mail clients would be able to use it. Can a template be used for
this?

The first question to ask is whether all the clients have Outlook. If they
do, you can send them a pre-addressed Outlook form template .oft file.
