outlook 2003 follow up folder not showing

The "For Follow Up" folder is a search folder. It should be created
automatically when you mark an item for follow up. It certainly works that
way for me.
 
Look under Search Folders. It's possible it just got removed from the
Favorite Folders list. If you find it under Search Folders, you can right
click the folder name and choose "Add to Favorite Folders"
