Outlook 2003 error after installing 2007

Problem statement:
After upgrading Office 2003 to Office 2007, my system is throwing a
mapi32.dll error when openning Outlook 2003 in the quick launch toolbar. The
upgrade program appended instead of replaced the Outlook shortcut on this
toolbar.

Expected behavior:
During the upgrade the program should replace the shortcut to Outlook 2003
on the quick launch toolbar with a shortcut to Outlook 2007.
 
That would depend on how the shortcut is actually created. Manually created
shortcuts are not affected.
Note that only one version of Outlook can exist on a machine at a time.

There is a problem in which Outlook 2003 is not always removed when
Outlook 2007 is installed. Microsoft has been rather unsuccessful in
tracking down this issue.
