Outllook 2002 custom forms

I have been working on a custom task form and I am having
problems with my final revisions.

I have redone the form 3 times. When i publish the form
then open for a new task it works but as soon as I save
and re-open it brings up the 2nd version form and not my
final version.

I have gone through my entire computer assuming that the
Outlook program is opening from a different source and
have deleted any test forms or any customized forms
previously worked on and nothing is helping.

Does anybody know what I maybe doing wrong?
