Option Buttons within an Online Form

I have created an online form with option buttons. How do I get people responding to the form to get more than one option button to stay marekd.
 
Use a radio button for each item
instead of an option button.

Hi Joyce,

Options buttons, aka radio buttons, are used to allow the visitor to select
one of 'n' items, i.e. like the push buttons on your cars radio you can only
select one.

If you need to allow for the selection of 'n' items then use checkboxes
instead.
