Option buttons not mutally exclusive under same group name

I am trying to setup a form with a yes/no questions. I have chosen radio
buttons (option buttons) for this and grouped the pair of them under the same
Group Name. However when I exit design mode and lock the form, both radio
buttons can be checked at the same time. I've checked and rechecked them and
even setup an entirely new document with only two option buttons and the
behavior is the same. Can someone tell me what I am doing wrong?
