One more question about a custom form

I have successfully created a custom form with fields prompting for specific
information by the users. However, the form disables the Attach icon on the
ribbon. We need the attach button to be active and I have been unable to
find a switch to enable it.

Any assistance would be appreciated.

Sounds like you removed the message/item body control from the form. You'll need to add it back by dragging the Message or Notes field from the Field Chooser to a custom page on your form.
