Numbered list gone awry

When I attempt to number a list, I highlight the list and click on the icon
in the toolbar. Instead of numbering that list, it numbers the whole
document up to 999 with a multitude of misalignments as well, including
changing the heading numbers.

I use 'control+z' to clear the extra numbers and misalignments, but I don't
know what is causing the document to renumber the whole document. Is there a
way to avoid having those extra numbers appear?
