New "Open With" problem

Following up from a few days ago with the problem in here that I
couldn't set "Edit With Dreamweaver" as a right-click option to HTML files :

If my default HTML viewer is IE then "Edit with Dreamweaver" appears in
the right click options.

If I change it to FF (which is what it should be, as I can't even print
properly using IE, but that's another story), the "Edit with
Dreamweaver" option does *not* appear.

If I change the default HTML viewer back to IE, then the option is back
again.

Now I can't manually go in and add Dreamweaver as an option (when the
default viewer is FF). I go through the dialog boxes, point to the
Dreamweaver executable, click ok, but it doesn't come up in the box, the
only ones are IE, FF, and Notepad, exactly the same as before I point to
the executable.

So somewhere along the lines something is working against something else...

Can someone help me set things up so

a) HTML files open with FF by default, and
b) There is an "edit with Dreamweaver" option in the right click menu.

I have made Dreamweaver the default editor in IE7 as well, so I can open
webpages directly into Dreamweaver from IE. These old instructions are
pretty much the same:
