new box opens up for microsoft details

I installed Outlook 2003. I must have done something incorrect when I
installed it. When I open it up, a new box opens up saying" Microsoft
Details"
Server name: NULL
Port: 3268
User name: NULL
Pass: __________
User Pass*
Do i have to uninstall outlook and reinstall it again?
 
Sounds like you have added the LDAP address book service. Remove it from
your mail profile if you don´t use that service.
