It may be possible if you activate UPnP on the router, but doing so raises
other security concerns, for example allowing users to set-up BitTorrent
clients.
The issue here is tht the H323 protocol was designed in the days when
security was not considered important, and every computer had an
Internet-visible IP address.
I would suggest looking at a more modern piece of software, e.g. Skype.
