You're asking in the wrong place. Access is a database program, and
Forms are data entry/editing screens within Access.
The format of a witness statement will vary from jurisdiction to
jurisdiction; there's no guarantee at all that a template for one
place would be suitable in another. I would suggest you contact an
experienced court clerk or court reporter in your area and ask what
they use.
