Need help with changing the way my contact card looks

I'd really like some help with changing the information contained on the page
when I pull up a contact. For example, buy defalt, it comes up with the name
address and phone # along with a few other things. I would like to add more
fields to include, something like, how many people work in that place or what
date I first contacted them, and what date I last service them. I designed a
form exacty how I want it but the form I made is not the one that comes up. I
would like to make my design be the defalt form used for my contacts. How
can I do that? Is it possible?
