Need help designing a custom form

Hello There

I'm trying to design a custom form in a contacts folder with some
requirements that include:
-a button to browse for a picture in my hard drive
-a button to browse for a attachment file my hard drive
Is it possible (and if so, how can I do it) to create these facilities
within a contacts folder?
