need a format to type addresses of people in my RV Club

I want to type a list of address, with phone numbers and emails of all the
people in my RV Club. I tried typing all these address free hand but it
looked awful, so someone suggested I type them on a form from WORD. I wanted
a beach theme as we live by the beach. Can anyone help? I just need the form
so I can type them up...probably 5 lines per family would suffice . And
perhaps if not a beach scene, then clip art of seahorses and starfish, fish
and dolphins....something to do with the sea or the beach
 
Obviously no one here is going to have a handy-dandy template to meet your
requirements. I would suggest you start playing with the formatting and
adding appropriate clipart. (I'd use Publisher but that's not a surprise to
too many people.)
