my message rules only run on some of the messages in my inbox

I have a set of message rules that only work sometimes. I can run them
manually and they sort things accurately, but then the next time a few
messages come in, some of them may be sorted and some may not. I can't
figure out why only some of them are working.

The same thing happens to me. Does anyone know what could be causing this?
Running Outlook 2003 with all the updates.

i.e. My virus software tags spam with "spam:..." in the subject body. Then
I have a rule that is set to send all emails with "spam:" in the subject to
send directly to junk or deleted folder. Works 50% of the time. As I am
compossing or sorting emails, 3 came into the Inbox just this morning. They
did not sort them to junk or deleted folder.
