My mail merge almost worked, but left out last name

I was so excited, that I got as far as this, almost everything worked.
Except all the letters left out the last name. I'm doing this in Word
2002, and my data source is in Excel 2002. The headings in the
spreadsheet start out like this
Last Name/Company Name, First Name, Co-signer,Address,City,State,Zip.
When I click on recipient list (data source) it also starts out with
first name only. Is the slash company Name the problem? Any help is
appreciated............Peter

Got a message about leaving out the extra writing, leave only Last
name.......it worked.....thanks..........Peter
