mail merge letter with multiple entries

I'm trying to set up a mail merge letter to be addressed to individual
companies that have been nominated for an award by one or more
nominators/companies. It reads like this:

«First Name» «Last Name»
«Company»
«Address»
«City», «ST» «Zip»
Dear «First Name»:

You have been nominated by «Nominator Name» of «Nominator Company», by
«Nominator Name» of «Nominator Company» and by «Nominator Name» of
«Nominator Company» to …..

I haven't seen any posts that directly address this type of arrangement
of set up. Any help in setting this up would be greatly appreciated!
