What's the recommended way to create multiple references to the same
footnote?

What happens if they are not all on the same page?

What happens if they are not all on the same page and I have fotnote
options set to renumber from 1 on each page?

I have a client information form that has several places where phone
numbers are requested. I would like to create a single footnote with
instructions for how to fill in their phone number and have all of the
phone number fields refer to it.

PS: I tried copying the footnote reference (the little superscript)
from the first reference, but this caused a new (identical) footnote
to be created.

I also tried using the Insert / Cross-reference facility. That works,
but the footnote reference does not come in as a superscript. I can
fix that with Ctrl+Shift+"+".

I was just wondering if there is a better way.

Lawfirms do this all the time. You create a bookmark to the footnote
reference and then a cross reference to that bookmark. It's a bit clunky but
it does work. You might need to superscript the font of the cross ref field
so it looks the same as the original cross reference.

That's what I did, but it is a bit clunky. I was hoping for a smooth
way to select an existing footnote when I do Insert / Footnote.

PS: The current scheme is not only clunky. It's also just poor
architecture.

1. If I delete the original reference, the footnote itself is deleted,
so all of the other references go nowhere. The right design would be
to make all references identical and only delete the footnote text if
all of them are deleted.

2. The cross-references function differently than the footnote
reference. You don't get the footnote text when you hover over the
reference, for example.

Thanks. I missed that last option (Footnote number (formatted)).

That fixes the need for me to manually convert it into a superscript,
right?

We still have the problem of the footnote text getting deleted if I
delete the original reference.

That's how Word works though - if you delete the reference you delete the
footnote. You should never delete a footnote from inside the footnote pane.
If there's a missing footnote the field will update to say "Error Reference
not Found" or something, and usually these are bold so they stand out from
the text.

The bookmark is built-in. You don't have to create it; you just select
"Footnote" in the Insert | Cross-reference dialog.
