Perhaps "Enhanced Pointer Precision" got enabled. Go to Mouse
Properties, Pointer Options, and UNcheck it.

Is this on a laptop? If so the pointer or pad may be going bad. I've
seen this happen on Dell's and IBM's.

Use a dull, porous surface or mousepad for any optical mouse or the
reflections can make the pointer jump.

you did not if it is while computer is in use or not in use.
if you are not using it, turn mouse upside down.
