monthly invoice

I'm needing to create a monthly invoice for several tenants in one building
that will change the date and the amount owed (which will be a formula of the
% of the whole amount) monthly.
 
Okay. Do you have a question for us?

First, this would be a report, not a form.

You will have to tell us much more about your database structure before we
could help you with this report. What have you tried so far? What tables
and fields are you using?

Much more information needed.
