How can one download images to month folders instead of day and month using
Vista
 
Do you mean using Windows Photo Gallery?
I don't think you can...
go to File - Options - Import
and check the available options for the folder name and file name.

In the newer Windows Live Photo Gallery, you have a few more options, like
(None), so you can save to the same folder.
