What is this and is it necessary in the start up? I took it off with
MSCONFIG but it comes back.

that is the microsoft synchonization manager. it is used for things like
syncing the active desktop web content and probably some other stuff.
 
Does it need to load at start up? If not, how to stop it?

it probably depends on what types of things you are set to sync. try
searching support.microsoft.com and google to see more about what it does,
maybe you can stop it from loading immediately if you turn off everything
that gets synchronized.
