Missing Notepad in context menu

Hi

Both my 'desktop > right click > new' menu and my 'Windows
Explorer > File > New' menu contain many items but not a new text
(notepad) document.

I'm sure there used it used to be there. How can I restore it, or
add such an item?

Adding an entry to HKEY_CLASSES_ROOT\Folder now gives the
required entry when right clicking on a folder in Windows
Explorer.

Adding an entry to HKEY_CLASSES_ROOT\Shell and to
HKEY_CLASSES_ROOT\Shell\Shell (both were already present) does
not seem to do anything - or I haven't yet worked out how it
works.

The context menus mentioned in my original enquiry are both
unaltered. [Each contains Briefcase, Microsoft Word Document,
Microsoft Office Binder, Paint Shop Pro Image, Microsoft Excel
Worksheet and more but NO New Text Document].
