missing menu bar in windows explorer

The menu bar in Windows explorer, that shows File, Edit,
View, Etc. has disappeared. I cannot use keyboard
shortcuts to access them either.

The menu bar is also missing in dial up networking and I
don't know where else.

I have not (intentionally anyway) changed any settings on
the computer.

Any Ideas?
