Messenger and SBS 2003

We've been using windows messenger for voice & video for our 7 locations. In the past 2 months we have set up a MS SBS2003 Premium Server at our main office. The network topology is cable modem into SMC Barricade router (with UPNP enabled) 192.168.0.xxx. The SBS 2003 server has 2 nics with ISA lodaded. Externall nic 192.168.0.225..internal 192.168.16.2. The client machines where previously able to send and rec. video and voice, now they can't. All client machines are dhcp with the SBS2003 as dns, gateway and dhcp controller. Of course normal log-on, text still works. I believe the problem is with the translation from external ip to local gateway 192.168.0.1 and then to server 192.168.16.2 to clients 192.168.16.xxx. Please advise on how best to reconfigure to get this to work
