Message form missing CC and Subject

For no reason that I am aware of my email message form no longer has the CC
and Subject captions or fields.

I have not edited the standard message form; but, I have opened a new
message and gone to "Tools/Forms/Design This Form" and the captions and
fileds are there.

What could have caused these items to disappear? More importantly, how do I
get them back?
