I am trying to create a custom message form to send to people outside my
organization who what more information about my consulting services. I have
been able to set up the form and have successfully sent it to myself. When I
try to send it to someone else in my organization for testing, they can fill
out the form but when they click reply, they get the error "the form you
selected could not be displayed". When I try to send it to someone outside
my organization, they do not see the form, just a blank email message. In
the actions tab of the custom form, I have Reply, Reply to all and Forward
set to Create form of type=Quote Request (my custom form).

The Microsoft Manual that I am using to help me set up this form provided as
an example of custom message forms, a client satisfaction survey. I assumed
from this example that the recipient of the message did not have to have the
form installed on his/her machine (or didn't have to have Outlook for that
matter).

I am running Outlook 2000, Internet only.

I'm trying to do something similar to yourself. Could you let me know,
please, the name of the Microsoft manual (with ISBN if possible) so
that I can follow the theory a little bit better than at present?

Microsoft Outlook 2000 Step by Step. ISBN: 1-57231-982-8. I don't think
this reference will help much because this book provides very limited
information about how to create forms.

A custom message form is not a good solution for this scenario, as you've already seen, because the recipient does not have access to the published reply form.

Depending on what you are actually trying to accomplish, a Word document attached as a file might be a better solution.
