merge it to microsoft office word

when I select merge with microsoft office word from one of my forms, it is
always missing the same field?

is there a way to correct this?

Try rebuilding the form. It may be that there is something corrupt. If you
have an old copy of the database, you might try substituting the form
temporarily.
