Ok. You've done the basics: create a table with those fields. I don't
know what "dos" is, but the rest should probably be Text fields; you
might want to change the field "Name" to some other name, since Name
is a reserved name.

If you have further problems, please post enough about them that
someone can understand what you need, and reply.
