Mark Index Entry>Mark All

In the 'Mark Index Entry' dialog, when I click 'Mark All,' only the first
instance of the relevant text in any given paragraph is marked. Is that
because an index only provides one reference to a given page, even if an
entry is marked multiply on that page, so marking mulitple instances in a
single para would be overkill?
