I don't know if this will be helpful, but here is the text of the emails I've
exchanged with the people at MAPILAB. I updated Norton Internet Security and
it helped, but then it happens periodically again and again.
So, to answer, NO, I have not really gotten this resolved, but this
methodology seems to have some temporary fix. If you can figure it out, I'd
love to hear how you did it.

If you have uninstalled our program it can not cause the problem.
Try to find out the reason in the following way:
1) Point to the Menu 'Tools'/'Options'/'Other'/'Advanced Options'/'Com
Add-Ins' and turn off all plug-ins you can see there.
2) Turn off all programs in ‘Com Add-In Manager’
3) Restart your Outlook
4) Point to the same menu and mark with a tick every program in turn,
restarting Outlook after each turning-on.
So you can find the program initiating the error.

"Error: "Managed MAPI Service Catastrophic Failure" with Norton Internet

Situation:
After installing Norton Internet Security or Norton AntiSpam, you see an
error message titled "Managed MAPI Service Catastrophic Failure" when opening
Outlook 2003. You may also see the text "Unknown error" in the error message
window.
Solution:
This problem was fixed by the latest Norton AntiSpam 2005 program update. To
install the update, start Norton AntiSpam or Norton Internet Security, and
then click LiveUpdate. Download all available updates, including the Norton
AntiSpam program update, and then restart the computer if prompted. For more
details on how to run LiveUpdate, see the document How to run LiveUpdate."

The MAPILAB program is the Outlook Mail merge program that enables Mail
Merge from the Outlook address book into the text of emails. I don't know
the version, I attempted to delete it.
