Making Payslip For Employees

Hey guys,

i want to make a form which makes payslips for employees depending on
what the user enters in the form(such as hours worked this week) Once
the user has entered something into this field such as 35(for hours).
In the form i will have employee details already come up via the
employee table records. In this table i have an hourly wage column so
the employees wage is already predetermined. So all i need is a text
box so when the user enters the hours worked the weekly wage will
appear below in another text box i will create called'net pay'. I have
tried setting up a text box for users to enter the hours worked but
when i create a new form and enter new hours worked, the number i enter
in the new hours worked text box appears in my old payslip as well. How
can i make the text box related to only one form because at the moment
when a user enters something into this field it becomes the case for
all the payslips i created before hand. Any help would be v.helpful :)

thanks for the reply pietlin

so i use dlookup on the text box i created for the users to enter the
amount of hours the employee has worked?
