Mail Merge with Publisher

I have my form, and my query created with a Yes/No selection. I am looking to
merge the records into a PUBLISHER document. I have seen the info on the Word
merge, but it HAS to be to Publisher. How do I do this?

I am looking also for a place on the form that I can enter dates for things
such as Date Ordered, Date Completed, along with a few more fields. I need to
be able to have more than one entry area for these, to record what a customer
does each year.

You should probably ask in a publish group. I don't even know if publish
supports mail merge at all....
