Macro for ASK and REF fields

I need to create a template with prompts for the user to type in information. I thought I could use the ASK field, and then the REF field to insert it in the text.

I then tried to record a Macro "AutoRun" to prompt the user to enter the information when the file is opened. However, during the recording when I press F9 for the fields, I have to enter information into the field pop-up window so I can stop the macro recording. The result is the macro automatically inserts the sample name and doesn't prompt the user when the file is opened.

How can I create this form for users to enter information and it be inserted in the appropriate places?

You need a template with the ASK and REF fields placed as required and saved
in it an AutoNew macro to update the fields.
When you create a new document the macro forces an update of the ASK field
and the REF fields
